if you have a myGov account with id verification, this can be done digitally and instant.

If you don't have myGov, or you don't wish to do it digitally, you can do it manually and post it in. forms can be free filled online and printed, or you can pick up a stat dec at the post office, or your local library (library should have it available free).

You will then need to get this witnessed at a JP, who usually have a community desk in shopping centre or library also (despite the list of acceptable witnesses, I NEED it to be witnessed by a JP, as I will contact them to verify)
